This privacy agreement is signed between Alacam Orthodontics (Address: Mustafa Kemal Mah. 2118. Cad. Maidan Business Center A Block, No:126 Çankaya/Ankara) and [Service Receiver] in the field of [Service Type].
In this agreement, [Service Receiver], referred to as the CLIENT, and [Service Provider] as the SERVICE PROVIDER.
The SERVICE PROVIDER agrees to act in accordance with the provisions of “The Contract for the Protection of Persons with regard to the Processing of Personal Data,” in particular, Law No. 6698 on the Protection of Personal Data, the provisions of the “Convention for the Protection of Human Rights and Dignity in the Application of Biology and Medicine,” medical professional ethics rules, and other ethical rules related to the subject, as well as the precautions requested by the CLIENT when using the information and personal data provided to or acquired by them in the course of providing services to the CLIENT’s clinic.
The SERVICE PROVIDER undertakes to not process or share personal data acquired during the provision of services to the CLIENT, including specially qualified personal data of patients and their relatives, without the CLIENT’s permission and beyond the intended purpose. This obligation continues indefinitely even after the service relationship ends.
This agreement covers not only personal data of patients but also special medical techniques applied by the CLIENT. In this context, the SERVICE PROVIDER agrees, declares, and undertakes to not share with anyone the special medical practices and techniques applied in the clinic and to act in good faith and honesty in accordance with rules of fair competition.
During and after the continuation of the service relationship, the SERVICE PROVIDER acknowledges, declares, and undertakes that any breach of the obligations set forth in this commitment will be subject to their legal and criminal liability, and they will unconditionally compensate for the material and moral damages incurred by the clinic and patients.
In matters not covered by this agreement, the provisions of “The Contract for the Protection of Persons with regard to the Processing of Personal Data,” Law No. 6698 on the Protection of Personal Data, the provisions of the “Convention for the Protection of Human Rights and Dignity in the Application of Biology and Medicine,” Labor Law, and other relevant legislation shall apply.
In case of any disputes arising from the implementation of this agreement, the courts and enforcement offices of Ankara are authorized.
This agreement has been read, signed, and a copy has been delivered to the parties. ……/……/………………
